Write Reflection Here:: My institutional IPPE rotation at the Long Term Care (LTC) provided me with an opportunity to observe, learn from and work with over five pharmacists and to participate in patient care activities under the guidance of an experienced preceptor over the last four weeks. I have been able to apply didactic information from school to the day-to-day activities as an Intern with emphasis on the development of professional maturity, judgment and skills. This practice experience encouraged me to use knowledge and technique gained in the classroom to interact with nurses on phone in LTC facilities and secondly to confidently work with the Pharmacists at my site. These experiences have reaffirmed my goal to be a pharmacist because I have become proficient in communication, gained confidence in my abilities in carrying out patient-centered care activities and to work in interprofessional teams. With everything I have learnt from my preceptor and her team of pharmacists, I have become more enthusiastic about my future and I look forward to the next phase of experiential education.

Please provide one example of an interprofessional observation and/or interaction that you had during this IPPE rotation in one to two sentences.: In the Longterm care where pre-packaged medications are processed and delivered to the nursing homes, the Pharmacist received a call from the Director of Nursing (DON) on the morning of August 10th, 2022 concerning a missing medication package that needed to be administered to a patient that morning.The pharmacist then discussed with his fellow Pharmacist and they both traced the package back to the Nursing home. A call was placed to the DON and she had to search again. A few minutes later, she confirmed the package was filed away in the wrong location.

During my block 1 IPPE rotations at a Longterm Care, one of my tasks as a Pharmacy Intern was to call the nursing facilities for clarifications on prescription orders sent to the pharmacy. These clarification calls were Pharmacist-Nurse type of communications.

I was surprised to find out that most of the nurses I spoke to either lacked communication skills or I'm hoping they were too busing to talk on phone so either rushed the call or decide to be rude.

Though I keep making excuses for some of these behaviors I experienced, I am still very disappointed because I had high expectations for them.

I feel this way because our Professors spend so much time equipping us with soft skills in class, in lab and through inter-professional seminars.

The best I can do now is to continue to be patient and calm during these calls, continue to practice communication based on respect and professionalism and with more practice I can further sharpen my communication skills as well as inter-personal skills.

P2 - FALL - IPPE Community Rotation - Field encounter
A patient called the Pharmacy needing his medication urgently but the Pharmacy had not received the script when the Patient called. The Pharmacist had to call the doctor's office and liaise between the provider and patient to get the script sent through to the Pharmacy. The team worked swiftly and efficiently within the shortest possible time to get the prescription processed and the medication ready for the patient. The patient was able to pick up via the drive-through service and was very appreciative of the prompt service provided

P2 - FALL - IPPE Community Rotation - Unique experiences 1
When I started my IPPE Community rotation, I had the opportunity to be the main Intern giving vaccinations.

Initially, I was nervous because I had not given any shots since I acquired my immunization certificate.

My preceptor went over the process with me a couple of times on my first day and the rest of the day I gave over 25 vaccinations. Currently I give over 45 vaccinations per day - mostly flu, COVID 19 booster and Shingrix and this has become like a second nature.

I became comfortable with the process, I gained more confidence each day and my technique got better per each vaccination. Mostly the feedback I get is that my technique is "quick and painless".

Though giving immunizations may not be a difficult task, it is also very easy to make mistakes so I double check each vaccine, double check patient's name and date of birth to ensure that I am giving the right patient the right vaccine.

P2 - FALL - IPPE Community Rotation - Unique experiences 2
This IPPE Community rotation has given me hands-on experience with Retail Pharmacy operations. I have had the opportunity to learn and understand the role of a pharmacist. Over the last 4 weeks I have worked together with an experienced Pharmacist and Preceptor to dispense prescription medications to patients and offer expertise in the safe use of prescriptions. We also conducted health and wellness screenings, provided immunizations and offered advice on healthy lifestyles.

I also went offsite with my Preceptor for flu clinics at a Senior Center and a Community Centers in 2 different towns. I felt very fulfilled on these occasions. Providing care especially to the geriatric population and the less privileged at the community center for immigrants, gave me a sense of purpose. I have no doubt being a Pharmacist and providing care is the career choice for me.

I have been able to apply the knowledge and skills learned in school to real patients, situations, and settings. I have improved upon my problem solving skills, communication and interpersonal skills with patients and other healthcare professionals. Multitasking and attention to details, I have realized are also very useful skills to have in a Community Pharmacy setting. I intend to continue to improve on these skills as I prepare for my career as a Pharmacist.

Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: During my ambulatory care rotation, one of the attending physicians wanted to know under what circumstance an ARB (losartan) be prescribed over ACE inhibitor (lisinopril) for an onset hypertension. The number one reason may be mostly due to reduced risk of dry cough with ARBs. This type of cough starts within 1-2 weeks of starting the medication.

Please provide one example of an interprofessional interaction that you had during this APPE rotation in one to two sentences.: During the poster session at the AACR annual meeting, I shared ideas with an MD and a nurse about the specialized targeted delivery system I presented for the treatment of triple negative breast cancer. This therapy will increase drug specificity and reduce cytotoxicity for patients using this mode of treatment.
